Update tests for location tuple and opcode field
...
Frame location is now a 4-tuple (lineno, end_lineno, col_offset,
end_col_offset). MockFrameInfo wraps locations in LocationInfo struct.
Updates assertions throughout and adds opcode_utils coverage.
