Added dedicated tests for CBC
...
In the process, the following changes are done to CBC code:
- `new` detects when a spurious extra positional parameter is passed
- `new` accepts 'iv' or 'IV' keywords, but not both
- the cipher object has both 'iv' and 'IV' attributes, for convenience
- `encrypt` and `decrypt` cannot be intermixed
- some ValueError exceptions have been converted to TypeError when
they refer to erroneuous API usage

To simplify, no RNG needs to be provided with PKCS1 encryption: the one belonging to each RSA key is reused.

Error detection is internally implemented in a simpler (and safer) way for PKCS1 OAEP decryption.
General fixes to documentation for PKCS1.

cleanup: Move modules to "lib/Crypto" subdirectory.
...
This will avoid the previous situation where scripts like the old "test.py"
get included accidentally in a release. It also frees us to put additional
build scripts in the top-level directory of the source tree.
